A Mayo Clinic study at the 2026 Menopause Society meeting found women on hormone therapy lost 11.9% weight on semaglutide vs 10.9% without. Observational, not proof.

Can hormone therapy help women lose more weight on Ozempic or Wegovy? A new real-world study from Mayo Clinic Florida, presented at The Menopause Society 2026 Annual Meeting on 20-24 October, suggests it might. Among women over 45 taking semaglutide for at least a year, those who were also on menopause hormone therapy lost slightly more weight than those who were not. The difference is small and the study is observational, so it does not prove that hormones cause extra weight loss. It is a useful signal, not a reason to start hormones for weight loss.
Researchers led by Dr Regina Castaneda at Mayo Clinic Florida reviewed electronic health records from February 2017 to December 2025. They studied more than 3,800 women aged 45 or older with a BMI of 27 or higher who had taken semaglutide or tirzepatide for at least 12 months, some with menopause hormone therapy and some without. Women with prior weight-loss surgery, active cancer or other obesity medicines were excluded. The groups were matched by age, starting BMI, earlier obesity drug use and type 2 diabetes.
For semaglutide (about 2,400 women), those on hormone therapy lost 11.9 percent of body weight at last follow-up versus 10.9 percent without it. At 12 months the gap was 11.0 percent versus 9.5 percent. Women on hormone therapy were also a little more likely to lose 20 percent or more of their weight (22 percent versus 18 percent).
For tirzepatide (Mounjaro, about 1,500 women), the picture was different. There was a small early edge at three months, but no meaningful difference in total weight loss later on. So the benefit, if real, appears mainly with semaglutide.

Many Indian women notice weight gain, especially around the belly, during perimenopause and after menopause, usually between ages 45 and 55. Falling oestrogen changes how the body stores fat and can make weight harder to lose. With semaglutide and tirzepatide now widely available in India, more women in this age group are asking whether these injections work as well for them, and whether hormones should be part of the plan.
This study offers reassurance that GLP-1 medicines work well in women over 45, with average losses of 10 to 12 percent of body weight in real-world use. That is meaningful for blood sugar, blood pressure, fatty liver and joint pain, which are common concerns for Indian women after menopause.
Because women were not randomly assigned, other differences between the groups could explain the result. For example, women who choose hormone therapy may be more engaged with their health, exercise more or follow up more closely. The abstract also did not report hormone type, dose of the injection, menopausal stage or how well women stuck to treatment. The researchers themselves call for larger prospective trials.
Menopause hormone therapy is not approved or recommended as a weight-loss treatment. It carries its own risks, including blood clots, and is not suitable for everyone, particularly women with a history of breast cancer, clots or certain liver conditions.

If you are a woman over 45 considering a GLP-1 injection, keep these points in mind. First, get a proper assessment, including BMI, waist size, blood sugar, thyroid, lipid and liver tests, before starting. Second, do not start or stop hormone therapy because of this study; discuss menopause symptoms such as hot flushes, poor sleep and mood changes with a gynaecologist on their own merits. Third, protect your muscle and bones. Eat enough protein, do strength training two to three times a week, and ask about calcium and vitamin D, since weight loss after menopause can reduce bone and muscle mass. Fourth, expect side effects such as nausea and constipation early on, and increase doses slowly under medical supervision.
Early evidence hints that combining semaglutide with menopause hormone therapy may give a modest extra boost in weight loss, but it is not proven and should never replace a decision made with your doctor. The most important step is a personalised plan that considers your menopause symptoms, other health conditions and goals.
