Quick Answer

A study of 256,000 adults found GLP-1 drugs cut type 2 diabetes risk by 31% versus metformin in prediabetes. Early data, but worth discussing with your doctor.

Can a GLP-1 medicine do more than metformin for people with prediabetes? A large new study presented at the American Heart Association Scientific Sessions 2026 suggests it might. Among about 256,000 adults with prediabetes and obesity, those taking GLP-1 drugs such as semaglutide or tirzepatide were 31% less likely to develop type 2 diabetes than those taking metformin. They were also about 1.5 times more likely to return to normal blood sugar. This is early, preliminary research, so metformin is not being replaced, but it matters for India.

What Happened

Researchers from Mayo Clinic reviewed health records from 104 hospitals and health centres in the TriNetX network, covering 2010 to 2023. They compared adults with prediabetes and obesity who took a GLP-1 medicine (semaglutide, tirzepatide, liraglutide or dulaglutide) with similar adults who took metformin. The two groups were matched for age, other health conditions and medicines. The average age was 52 and about three in four patients were women.

On average, patients were followed for roughly three years. Compared with metformin, the GLP-1 group had a 31% lower risk of developing type 2 diabetes and a 41% lower risk of death from any cause. Heart attacks were 15% fewer, strokes 20% fewer, heart failure 25% fewer and atrial fibrillation 8% fewer.

Important Limits of This Study

This was an observational study, which means it cannot prove that GLP-1 drugs caused these benefits. The data came from diagnosis codes and prescriptions, so doctors do not know how regularly people took their medicine or at what dose. All GLP-1 drugs were grouped together, and the findings are an abstract that has not yet been peer reviewed. The study authors themselves say metformin remains a safe, low-cost, first-line choice, and that this is not about replacing one medicine with another.

An independent heart expert commented that the benefit is likely linked to weight loss, and that GLP-1 drugs could become an add-on option for prevention, with cost and access being important factors.

Why It Matters for Indian Patients

India has a very large prediabetes burden, and many Indians develop diabetes at a lower body weight and younger age than people in the West. Belly fat, fatty liver and a family history of diabetes raise the risk further. Many people are told to just exercise and eat less when their HbA1c or fasting sugar is in the prediabetes range, and some are given metformin. Studies like this are part of a growing conversation about treating the weight problem behind prediabetes earlier.

Cost is the big question. GLP-1 medicines cost far more per month than metformin, which costs only a few hundred rupees. Generic semaglutide has made treatment more affordable in India, but it is still a long-term expense, and weight often returns if the medicine is stopped. For many people with prediabetes, lifestyle change with or without metformin will remain the right first step.

What You Should Do Next

If you have prediabetes, do not stop or change any medicine on your own because of this news. Ask your doctor for a full check of your HbA1c, fasting sugar, cholesterol, liver health and body weight. If you also have obesity, ask whether a GLP-1 medicine could be considered alongside diet, a daily walk of 30 to 45 minutes and strength exercise.
