Quick Answer
Real-world GLP-1 weight loss is about half of trial results because many people stop early or never reach full dose. Here is what Indians should know.

Why do many people lose far less weight on Ozempic, Wegovy or Mounjaro than the headlines promise? A new review of the evidence says the medicines are not failing. The gap mostly comes from how they are used in daily life: people often stop early, and many never reach the full dose that was used in clinical trials.
A narrative review by Dr Zachary Bloomgarden of the Icahn School of Medicine at Mount Sinai, published in the Journal of Diabetes in July 2026, compared results from large randomised trials with results from real-world studies of semaglutide and tirzepatide.
In trials, people without diabetes lost roughly 15 percent of body weight on semaglutide 2.4 mg and about 18 to 21 percent on tirzepatide at higher doses. In real-world studies that counted everyone who was prescribed the drug, average loss was only around 4.5 to 7 percent of body weight, roughly half of the trial figure.
But among people who stayed on treatment, the picture changed. In one real-world study of persistent users, semaglutide users lost about 14 percent and tirzepatide users about 16.5 percent, close to trial results.

The review points to two main reasons. First, dose. In a large Danish registry of more than 110,000 semaglutide users, only about 13 percent ever reached the 2.4 mg dose. Weight loss rises steeply with dose, so staying on a low dose means a smaller result.
Second, stopping. In trials about 1 in 10 people drop out in the first year. In real life, between 20 and 50 percent stop within the first year because of nausea, cost, supply problems or loss of motivation.
This is a narrative review, not a full systematic analysis, and most real-world studies last 12 months or less. Most trials are also funded by the drug makers. So the numbers are best treated as a guide, not a promise. Blood sugar control in people with type 2 diabetes was much closer to trial results than weight loss was.
In India these medicines are expensive, often costing thousands of rupees every month. Many people buy a few pens, feel nausea, skip doses or stop when the budget gets tight. That pattern is exactly what leads to disappointing results, and it can also lead to weight regain after stopping.

Indian patients also tend to develop diabetes, fatty liver and heart disease at lower body weights than Western patients, so even a modest loss can help. But the benefit depends on staying on treatment and reaching the right dose under supervision.
Do not judge the medicine by the first month. The dose is usually raised slowly over several months, and the full effect takes time.
Tell your doctor early about nausea, constipation or reflux. Small changes in meal size, protein intake and dose timing often fix these problems so you do not have to quit.
Plan the cost before you start. Ask what the monthly expense will be at your target dose for at least 12 months, and discuss lower-cost options with your doctor if needed.
Combine the medicine with protein-rich meals, daily walking and strength exercise. Never buy injections from unverified sellers or change the dose yourself.
