Retatrutide, Eli Lilly's experimental once-weekly injection, lowered blood sugar (A1C) by about 2 percentage points and helped adults with type 2 diabetes lose up to about 16.8% of their body weight in a large phase 3 trial. It is not yet approved anywhere, including India, so it cannot be prescribed today, but the results show where GLP-1 treatment is heading.
In the TRANSCEND-T2D-1 trial, 537 adults with type 2 diabetes were randomly given retatrutide at 4 mg, 9 mg or 12 mg once a week, or a placebo injection. They had A1C between 7% and 9.5% and were not taking other diabetes medicines. Doses were raised slowly every four weeks, and results were measured at 40 weeks.
On the highest dose, A1C fell by roughly 2 points from a starting average of 7.9%. About 9 in 10 people on retatrutide reached an A1C below 7%, the usual target for good control, and up to 85% reached 6.5% or lower. People on the 12 mg dose lost about 36.6 pounds (around 16.6 kg) on average, and their weight was still falling at 40 weeks. Triglycerides, bad cholesterol, blood pressure and waist size also improved.
Semaglutide (Ozempic, Wegovy) works on one gut hormone pathway called GLP-1. Tirzepatide (Mounjaro) works on two, GLP-1 and GIP. Retatrutide adds a third, the glucagon receptor, which may raise the body's energy use and help the liver burn fat. That is why it is called a triple agonist and why researchers are watching it closely.

As with other drugs in this class, the most common problems were stomach related. Nausea affected roughly 16% to 27% of people on retatrutide, diarrhoea 19% to 26% and vomiting 15% to 18%, mostly during dose increases and mostly mild to moderate. A small number of people, 4.5% or fewer, reported an odd skin sensation called dysesthesia, and a few had urinary infections. Between 2% and 5% of people stopped the drug because of side effects. Earlier studies also showed a rise in heart rate that settled with time.
Importantly, long-term effects on the heart, kidneys and liver are still unknown. Larger and longer trials are needed before anyone can say how safe it is over many years.
India has more than 100 million people living with diabetes and is often called the diabetes capital of the world. Many Indians develop type 2 diabetes at a lower body weight and younger age than people in the West, and carry more fat around the belly. A medicine that controls sugar and reduces weight and fat around the organs together could be very useful here.
At the same time, a few cautions matter. Retatrutide is still under study and is not sold in India. Online sellers offering retatrutide, sometimes called research peptides, are not approved medicines and may be fake, wrongly dosed or unsafe. Please do not buy these.

If you have type 2 diabetes or obesity, you do not need to wait for new drugs. Approved options such as semaglutide and tirzepatide are already available in India and have strong evidence behind them. A doctor can check your A1C, kidney function, thyroid history, pancreas history and other medicines before suggesting what suits you.
Keep up the basics too: regular meals with enough protein, daily walking or strength exercise, good sleep and regular blood sugar checks. These make every medicine work better and help you keep the weight off.
